• 2009-2010 Student Handbook - Top page must be signed and returned to the front office.
• Mustang Merchandise Order Form

• NUSD - Athletic Release Form (http://do.nusd.org/forms/Athletic%20Release%20(writable).pdf)
     This NUSD Athletic Release Form must be completed (including doctor signature) and turned in the SMHS Athletic Director (through front office) prior to participation in ANY school athletic practice or game.

• NUSD - Driver Authorization Form (http://do.nusd.org/forms/Driver%20Authorization%20(writable).pdf) 
  Driver Authorization Form (along with DMV form linked below) must be completed and turned into SMHS Front Office in order to drive students other than immediate family members.
• DMV - Request for Record (http://www.dmv.ca.gov/forms/inf/inf70.pdf) 
  Request for driving record from the DMV to be included with NUSD form above. This costs $5 and takes minutes at the local DMV office.
  

• Tutoring Application - The form required to become a tutor at San Marin.
• NUSD Work Permit.- Required for students to work.  Processed at San Marin.
